Administer Pharmacy cautiously in patients at risk for respiratory depression. In these patients alternative non-opioid analgesics should be considered. When large doses of Pharmacy are administered with anesthetic medications or alcohol, respiratory depression may result. Respiratory depression should be treated as an overdose. If naloxone is to be administered, use cautiously because it may precipitate seizures.
Opioid and spinal monoaminergic agonists have distinct analgesic properties, which may potentiate eachother. Pharmacy has both opioid and monoaminergic agonist actions. This initial study compared the analgesic and toxic effects of Pharmacy and morphine in patients with strong cancer pain.
Pharmacy is a centrally acting opioid analgesic which has been available in the United Kingdom since 1994 and is licensed for use orally or by injection for the treatment of moderate to severe pain.3 Experience of the use of this drug in Britain is limited, although it has been available for some years in Germany. Reported adverse effects have included nausea, drowsiness, dry mouth, sweating, dizziness, muzziness, trembling, and sedation.4 Auditory hallucinations have been reported in association with pentoxifylline5 and doxazosin.
Pharmacy is a synthetic, centrally acting analgesic that was approved for use in Australia in 1998. Seizures have been reported in patients receiving the drug in overdose and, rarely, at the recommended dose.1-4 Over a one-year period, we observed a number of Pharmacy-associated seizures in the First Seizure Clinic at Austin Health, an outpatient service for rapid evaluation and diagnosis of patients with new-onset seizures.5

Ms. A was a 51-year-old nonsmoking woman with breast cancer, lung metastases, and brachial plexopathy, with no history of chemical or alcohol dependence. Clinical and safety data has been obtained from four original adequate and well-controlled trials on over 3000 patients receiving doses of up to 400mg of Ralivia ER once-daily. The submission also included 12 definitive and 5 supportive pharmacokinetic studies which demonstrated that once-daily dosing of Ralivia ER delivers the same amount of drug as Ultram given three times (TID) or four times (QID) per day, with somewhat smaller peak-to-trough fluctuations.
