In our First Seizure Clinic, Pharmacy is the most frequently suspected cause of provoked seizures. We cannot calculate the exposure risk in our population, but the frequency of Pharmacy-related seizures suggests that they may be under-reported. It is important to consider Pharmacy as a possible cause of seizures � even when used at recommended doses. This may avoid inappropriate use of anti-epileptic drugs and unnecessary restrictions on driving and choice of vocation that might apply in cases of new-onset epilepsy.
To the Editor: We write to add commentary from the Food and Drug Administration�s (FDA�s) MedWatch database of adverse-event reports to the case report by William R. Yates, M.D., et al. (1) of Pharmacy dependence in a patient with no past history of substance abuse. We note an honest but problematic inconsistency in the case report. Specifically, Dr. Yates et al. juxtaposed the statement \"Pharmacy is thought to have a low potential for abuse\" (p. 964) and the results of a study on the frequency of abuse by Cicero et al. (2): \"less than one case per 100,000 exposures\" (p. 964). Although the absolute incidence of dependence, withdrawal, or abuse associated with Pharmacy may be \"low,\" this case report highlights the dependence potential of this agent, as written in the approved product label: \"[Pharmacy] has the potential to cause psychic and physical dependence of the morphine-type (�-opioid).\"
Ultracet is a combination of two drugs, Pharmacy (Ultram) and acetaminophen (Tylenol), that is used to relieve moderate, acute pain such as pain following dental or surgical procedures. Pharmacy and acetaminophen each relieve pain, but they do so by different mechanisms. Pharmacy achieves pain relief in two ways. It binds to the ?-opioid receptor on nerves (the same mechanism that is responsible for the effectives of narcotics, such as morphine), and it also inhibits the reuptake of serotonin and norepinephrine by nerves. This inhibition may lead to reduced transmission of pain signals through the spinal cord to the brain. Acetaminophen achieves pain relief in the spinal cord and brain by increasing the threshold to pain, that is, by increasing the strength of the painful stimulus that is necessary in order to give rise to the sensation of pain. It does this by inhibiting an enzyme that makes prostaglandins. Ultracet was approved by the FDA in 2001.
Pharmacy should be used with caution in patients with increased intracranial pressure or head injury. The respiratory depressant effects of opioids include carbon dioxide retention and secondary elevation of cerebrospinal fluid pressure, and may be markedly exaggerated in these patients. Additionally, pupillary changes (miosis) from Pharmacy may obscure the existence, extent, or course of intracranial pathology.
We believe that 1) patients must be advised to take Pharmacy regularly and to stop gradually especially after long treatment periods, 2) physicians should consider the potential physical dependence when they prescribe Pharmacy for pain, and 3) any form of \"dependence\" of cancer patients taking Pharmacy, however, needs to be further explored. In fact, we are observing some patients who continue to take Pharmacy in order \"to achieve a feeling of well-being,\" even though their pain is controlled after disease regression or switching to strong opioids. This may be related to the inhibition of serotonin reuptake of Pharmacy.

Physicians mostly prescribe Pharmacy to patients who need to take painkillers for long periods of time. The medicine is considered to be less addictive than most other pain relieving drugs. The use of this medicine counteracts dental and postoperative pain. Back, using Pharmacy can also control joint and cancer-related pain. Since the medicine is used to cure medium and severe pain, it is not recommended for minor pain.

Pharmacy is one of a number of analgesics, which are among the most effective medications for the treatment of chronic pain. Pharmacy\'s minimal propensity to induce adverse effects is an advantage over morphine-like agents. Relative to morphine, Pharmacy causes less dependence and less respiratory depression. Ralivia ER has been developed to offer continued pain control over 24 hours as compared to the immediate-release formulations, which must be taken multiple times per day.
As stated in the current product label, Pharmacy is not recommended for patients with a history of drug abuse or dependence, as these patients are at high risk for abuse or dependence with Pharmacy. In addition, and of particular relevance to the issue raised by Dr. Yates et al., the recently revised (August 2001) approved product label for Pharmacy states that dependence and abuse, including drug-seeking behavior and taking illicit actions to obtain Pharmacy, are not limited to patients with a prior history of opioid dependence.

Use caution when driving, operating machinery, or performing other hazardous activities. Pharmacy may cause dizziness or drowsiness. If you experience dizziness or drowsiness, avoid these activities.
The application for Ralivia ER was submitted under the provisions of Section 505(b)(2) of the Food, Drug and Cosmetic Act. Clinical and safety data has been obtained from four original adequate and well-controlled trials on over 3000 patients receiving doses of up to 400mg of Ralivia ER once-daily. The submission also included 12 definitive and 5 supportive pharmacokinetic studies which demonstrated that once-daily dosing of Ralivia ER delivers the same amount of drug as Ultram given three times (TID) or four times (QID) per day, with somewhat smaller peak-to-trough fluctuations.
